Setting Up Services
A phone call to the office is
all we need to initiate a referral. One of our trained office staff will then set up an appointment with the
client and/or family to meet at the home or care site to do an assessment.
Case Assessment
One of our trained office staff will go to the home to meet and
introduce ourselves to each new client & their families. This helps us to be sure that we can direct the caregiver to
the right location, and also helps us to determine exactly what services are needed so that we may make appropriate staff
assignments. We also use this visit to share any additional information we have with clients or families that might make
the client more comfortable and the caregiver's job a little easier. CHCS is here to meet your needs, and as needs
change, so does your service. Therefore, we do not enter into contracts with our clients, but simply have a financial agreement
stating our rates, terms and conditions for payment.
While we are getting acquainted with the client, we are also asking questions about preferences, daily routines,
and special needs. These are all recorded on an assignment sheet so that our caregiver has an opportunity to know as much
as possible right from the start.
We will also supply you with a "Client Handbook". A wonderful tool and reference
guide outlining our services and standards of service.
When will service begin
Once our case assessment is complete, we will begin working diligently on the process of finding
the perfect caregiver for you. We will contact our caregiver by phone to accept the assignment and will discuss your needs,
preferences and duties. Most of the time, a day or two is ample notice, and we have often been able to find someone within
hours. We will make every effort to care for our client's individual needs.
